Part Two

A Part Two application form is e-mailed to you after successfully completing Part One of the application process.

The full application should be submitted within an interim period no longer than 3 months from Part One acceptance.  If this does not occur, the Part One application must be resubmitted.

The full application requires the Principle Investigator to sign a Material Transfer Agreement to ensure that the storage, use and disposal of the samples is in accordance with the HTA Codes of Practice and terms of the ethical approval.

Part Two application is e-mailed to enquiries@ethicaltissue.org. You are also required to sign a duplicate copy, which together with a signed copy of the Material Transfer Agreement is sent by post to:

The full application will be forwarded to members of the ETHICAL TISSUE Independent Scientific Advisory Committee for review. Based on the advice of the Independent Scientific Advisory Committee, ETHICAL TISSUE will approve the application.
