There are several ways you can notify us if you have changed your Bradford address, mobile or house telephone number

Please select your preferred method from those below, but if you have changed address please ensure it remains in our practice area: BD1, BD3, BD5, BD7, BD8, BD9, BD18. If it is outside our area you can find other local GPs on the NHS Choices web site: www.nhs.uk/Pages/homepage.aspx

1. INTERNET - Click here to open our web form for contact detail changes

2. E-MAIL - Send an e-mail to bradfordstudenthealthservice@bradford.nhs.uk

Include:

  • Your full name and date of birth (must be date of birth not University ID number)
  • Tell us your old BRADFORD address, mobile and home telephone number
  • Tell us your new BRADFORD address, mobile and home telephone number
  • We will e-mail an acknowledgement of receipt

3. TELEPHONE - Monday to Friday between 10am and 5pm you can telephone (01274) 234979 and ask the Receptionist to update your records

4. COME IN to our reception and ask for a change of address form. Reception is open Monday to Friday, 8.30am to 5pm

If you have changed address and have been referred to a hospital, make sure you inform them too or you will miss your appointment - you can contact our Medical Secretaries for help with this
