JOINING THE PRACTICE
To register with the practice you must be either:
• A full time student at the University of Bradford
• Or a member of staff at the University of Bradford resident on campus
• Or a full time student at Bradford College
• Or an immediate family member of a registered student
AND
• Live in the Practice Area (BD1, BD3, BD5, BD7, BD8, BD9, BD18)
You can register by downloading and completing our registration forms, or you can come into the Practice and complete the forms. This is a simple process and takes a short time only, and once registered you are immediately able to use our services. Graduates who remain within the Practice Area are welcome to stay with the practice
IMPORTANT NOTE
If you registered with us previously, for example last term, and haven't
registered with another doctor between now and then, for example
your home doctor, then you will still be registered with us and don't
need to register again, but you do need to inform us of your new
address (open our change
of address page)

5pm and ask for a Registration Pack to be posted to you, which you
can complete and post back to us, or drop into Reception

your full name and postal address including postcode, put registration
pack as the subject line
