JOINING THE PRACTICE
To register with the practice you must be either:
• A full time student at the University of Bradford
• Or a member of staff at the University of Bradford resident on campus
• Or a full time student at Bradford College
• Or an immediate family member of a registered student
AND
• Live in the Practice Area (BD1, BD3, BD5, BD7, BD8, BD9, BD18)
You can register by downloading and completing our registration forms (on our Register page), and bringing them into the surgery, or you can come into the surgery and complete the forms. This is a simple process and takes a short time only, and once registered you are immediately able to use our services. Graduates who remain within the Practice Area are welcome to stay with the practice
CAR PARKING
We have very limited car parking. Wheel clamping is in operation in
adjoining car parks so beware
NON-NHS WORK
Certain areas of medical work are not available free from the National
Health Service which means you will be charged a fee. Examples are
: medical reports, certificates, and some travel vaccinations. A
list of private fees is available at Reception
DISABLED PATIENTS
Full wheelchair access is available. Please inform staff when booking
appointments or on arrival
REPEAT PRESCRIPTIONS - (01274)

Prescriptions for medicines taken regularly can often be obtained without
seeing the doctor as long as the doctor has agreed to this in advance.
However, your illness will need to be monitored by, at least annually.
You can request your repeat prescription be either bringing in or
posting your prescription list, or by telephoning Reception on (01274)
235227 between 10am and 2pm Monday to Friday. Please allow one working
day for us to process your request. Click

PRESCRIPTION FEES & EXEMPTIONS
NHS prescriptions are charged for by the pharmacist, however, they
are provided free if:
• it is for contraception
• you are under 19 and in full-time education
• you receive income support
• you are pregnant or suffer from certain medical conditions
If you are on a low income, you may be able to obtain an exemption
certificate by applying on a form HC1 available from your doctor, pharmacist,
dentist, optician or the Benefits Agency
People who have to pay for more than 3 prescription items in 3 months, or 14 items in 12 months, could save money by buying a Prescription Pre-payment Certificate (PPC). From 1 April 2008, the charge for a single prescription item is £7.10, whereas a 3-month PPC will cost you £27.85 and a 12-month PPC £102.50. CHANGE OF ADDRESS
Please help us to provide continuing care for you by informing us of
any change of address or telephone number promptly. Likewise, please
inform the hospital if you are under their care, or awaiting an appointment.

COMPLAINTS & SUGGESTIONS
We operate a complaints procedure in line with national guidelines.
If you have a complaint about the service we have provided please
ask at reception for the complaints leaflet which outlines how you
should proceed. We are always happy to receive your comments and
suggestions for improvements to the way we run our service
CONFIDENTIALITY
All information you give to anyone in this Practice is considered to
be confidential. Where personal data is stored electronically we
adhere to the requirements of the Data Protection Act. In the course
of everyday patient care and administration your data may be shared
(in confidence) with specific NHS organisations, for more detail
see our Data Protection Notice in the surgery, or on our Advice
to users page. All employees have access to patient
data in relation to their role, and have signed a confidentiality

PATIENT'S RESPONSIBILITIES
It is our aim is to treat you with respect, care, and courtesy, and
we ask that you do the same. Please be on time for appointments,
and let us know if you cannot attend. Patients who are abusive or
violent will be removed from our list and Bradford
& Airedale Primary Care Trust will make alternative arrangements
for their healthcare
CLINICAL SERVICES
CONTRACEPTIVE ADVICE AND SEXUAL HEALTH
Please feel free to discuss with the doctor or practice nurse any concerns
you may have about sexual health, contraception, sexually transmitted
diseases and terminations of pregnancy. We offer completely confidential
advice and guidance. Please note that new patients must have their
first contraceptive review in a routine doctor's appointment,
they can then see one of our nurses for future contraceptive consultations
IMMUNISATION AND TRAVEL
Our nurses run weekly comprehensive travel advice and immunisation
clinics. Please remember to plan well in advance of your travels
(at least 6 to 8 weeks if possible), so that you have time to receive
any necessary immunisations - it is never too early to come and
discuss your travel plans with the nurse! Read

CERVICAL SMEAR TESTS
Our practice nurses run regular smear test clinics. All females over
25 years of age who are, or have been sexually active are advised
to have a cervical smear test every three years. Please note that
routine smears are no longer required for patients under 25, unless
there has been previous abnormal results
DIABETES
Our diabetes care is provided by the doctors with the help of our practice
nurses. We also work closely with the hospital's Diabetes
Specialist Nurse
ASTHMA
Routine asthma care is provided by our practice nurses, with the aim
of educating you about asthma, and helping to improve your control
and manage symptoms. Please remember to bring all inhalers, diaries
etc. to your appointment
MINOR SURGERY
A variety of minor surgical procedures can be undertaken in the Practice
after consultation with one of the doctors
MATERNITY
SERVICES (PRECONCEPTION, ANTENATAL AND POSTNATAL CARE)
We offer pre-conception, full ante-natal and post-natal care. Advice
and check-ups are carried out by the doctors and midwives
CHILDREN
We offer full Child Health Surveillance (development checks and immunisations)
to children of registered patients. We have an attached Health
Visitor, Ann Terry
DENTAL AND OPTICAL CARE
We do not provide dental care or optical services, but you can find
