Research title: Optimizing the transition from home to hospital: Enhancing three-way communication to improve the quality of dementia care.

Project: Optimising the transition from home to hospital: Enhancing three-way communication to improve the quality of dementia care. 

Though I am originally from Vancouver, Canada, I have been living in the UK and working on patient safety and quality improvement projects for several years. While improving hospital care has been a long-standing professional interest of mine, the real draw of this project for me was the opportunity to improve care for people living with dementia. This project focuses on improving the quality and safety of care that provided for people with dementia while they are in the emergency department.

Previous research has shown that the emergency department can be a stressful, confusing, and disorienting place for people with dementia and the people who support them. Using a mix of qualitative and quantitative methods, this project aims to build a better understanding of the current experience of accessing care in the emergency department to identify key safety risks and opportunities for improvement. Ultimately designing an intervention, which will then be trialled in emergency departments.
